In order to release Maryland Fund checks for Breeder, Owner and
Stallion bonuses, the MHBA must have the Payee's Tax Identification
number on file. The W-9 Form can be used by individuals, partnerships,
corportations, etc. to accomplish this.
However, if the payee is a partnership or involves more than one
owner/breeder, then an Ownership Disclosure Form must also be completed.
This form designates one individual/entity as the responsible party
to receive bonus payments. It also ensures that the breeder (or
owner) of record is directly identified as the holder of the corresponding
tax identification number.
This form MUST provide the names, Tax Identification
numbers, and signatures of all owners/breeders involved.
